All-on-4 Full-Arch Dental Implants

Losing most or all of the teeth in an arch changes more than your smile. Chewing becomes limited, speech can be affected, and loose dentures often make daily life uncomfortable. All-on-4 is a full-arch implant approach designed to give a fixed set of teeth supported by just four implants per arch, rather than one implant per missing tooth.

What Is All-on-4?

In the All-on-4 concept, four implants are placed in the jaw to support a full-arch bridge. The two front implants are usually placed upright, while the two back implants are angled. Angling lets the dentist anchor into the denser bone at the front of the jaw and place longer implants, which can reduce the need for bone grafting in the back of the jaw, where bone is often thinner and closer to the sinus (upper jaw) or nerve (lower jaw).

Who Is a Good Candidate?

  • Adults missing most or all teeth in one or both arches, or with remaining teeth that cannot be saved
  • Patients with enough bone in the front of the jaw (confirmed on a 3D scan)
  • Non-smokers, or those willing to stop, since smoking raises implant failure risk
  • People whose general health conditions, such as diabetes, are well controlled
  • Patients who are committed to thorough daily cleaning and regular reviews

Bone quality, active gum disease, certain medications and heavy grinding habits can change the plan. These are assessed at consultation.

Treatment Journey

The Treatment Journey

  • Assessment and planning: clinical exam, X-rays and a 3D scan (CBCT) to map bone, sinus and nerve positions.
  • Preparing the mouth: hopeless teeth are removed and any infection or gum disease is treated.
  • Implant surgery: four implants are placed under local anaesthesia, with sedation options for anxious patients.
  • Provisional teeth: when the implants are stable enough, a temporary fixed bridge may be attached within a day or two. This is called immediate loading and is not possible in every case.
  • Healing: the bone fuses with the implants over roughly three to six months, during which a softer diet is advised.
  • Final bridge: a custom, stronger prosthesis is made and fitted once healing is confirmed.

Aftercare and Long-Term Maintenance

  • Clean daily with a water flosser or interdental brushes as advised
  • Wear a night guard if you clench or grind
  • Attend reviews and professional cleanings, usually every 3–6 months
  • Report soreness, bleeding, looseness or a change in bite early
